Funding through National Disability Insurance Scheme (NDIS)

If the participant's NDIS plan is agency managed and they would like us to make a claim on the NDIS Portal, please provide the following information:

• The participant's name, date of birth, address and NDIS Participant No.
• The Support Budget Category
  Our products are usually funded from the Assistive Technology budget.
  And in some cases participants can claim from Core Supports - Consumables - Daily Adaptive Equipment. low risk technology
• The amount of funds currently available in this support budget
• The Support Item Name/s (if you have this information)
• List the items you wish to purchase
